PRE-REQUISITES for our classes

1.  Complete our 3 page application.

2.  Medical “Clearance completed and signed by your physician.

      You must have a TB clearance which includes a:

      1. Negative TB skin test within the last 6 months or

      2. Negative Chest x-ray within the last year.

      Our 2 forms must be completed and signed by Physician

3.  Current CPR Card – Adult, Child, Infant + AED

4.  No Felonies in the last 5 years

     - Fingerprints will be submitted to AZBN with

       application for AZ Certification.

5.  Proof of Citizenship or Legal Residency in U.S.

          (Birth Certificate, Pass port, Green card etc.)

6.  Call to Attend a Prescreen Interview at the school to Register.

7.  Pay $120.00 Registration fee (Includes Textbook)
